Upon booking your initial consultation, you will receive your new patient intake and consent form via email. It is recommended that you complete this form no later than 24 hours prior to your consultation.

Cancellation Policy

Because we reserve and hold your time slot, we require at least 24 hours notice for all cancellations. If less than 24 hours of notice is given or an appointment is missed, we reserve the right to charge you 50% of your consultation cost.
